Disclaimer: This guide is for general informational purposes only and does not constitute legal, financial, or medical advice. Health insurance rules for foreign residents in Korea change often, rates are reset every year, and how a rule applies depends on visa status, employment, and length of stay. For an individual case, contact the National Health Insurance Service (NHIS) on 1577-1000, extension 6 for English, or 033-811-2000 directly. Questions about employer obligations can also go to the Ministry of Employment and Labor on 1350, and visa questions to the Immigration Contact Center on 1345.
This guide covers: the two ways a foreign teacher ends up in Korea's National Health Insurance and which one applies, what an academy is legally required to do and by when, the 2026 premium rate and long-term care rate with a worked payslip example, the six-month rule that makes regional enrollment compulsory, when private insurance can replace national coverage, what the insurance pays for and what a visit actually costs, the card, app, and English phone line, what to do if an academy has not enrolled a teacher, how dependents are added, and what happens on changing jobs or leaving Korea.
Two ways in: workplace or regional
Short answer: Korea's National Health Insurance has two kinds of member. A teacher employed by an academy is a workplace subscriber (직장가입자), and the premium is split 50/50 with the academy. A foreign resident with no qualifying employer becomes a regional subscriber (지역가입자) after six months in Korea and pays the whole premium alone. Every E-2 teacher at a registered academy should be in the first group from the first day of work.
The distinction matters because the two categories differ in cost, in who files the paperwork, and in what happens when a payment is missed. Most of the trouble foreign teachers have with health insurance comes from slipping between the two without realizing it.
| Workplace subscriber (직장가입자) | Regional subscriber (지역가입자) | |
|---|---|---|
| Who | Employees of any workplace with at least one worker | Residents who are not workplace subscribers or dependents |
| When it starts for a foreigner | The day employment begins | The day after six months of residence, for most visa types including E-2 |
| Who enrolls | The employer reports it within 14 days | NHIS enrolls automatically |
| How the premium is set | Percentage of monthly wage (7.19 percent for 2026) | Points based on income and property, with a floor at the average premium for most foreigners |
| Who pays | Employee 50 percent, employer 50 percent | The member pays 100 percent |
| How it is paid | Deducted from salary | Paid in advance, by the 25th of the month before |
Coverage itself does not differ. NHIS states that foreign members receive the same benefits as Korean citizens, whichever category they are in.
Workplace enrollment: what the academy has to do
Short answer: An academy that employs a teacher must report the teacher to NHIS within 14 days of the start date, and eligibility runs from the day employment began, not from the day of reporting. There is no exception for probation, for a missing alien registration card in the first weeks, or for a teacher who says they do not want it.
This is the obligation that protects a new arrival during the first months, when clinic visits for colds, allergies, and minor injuries are most likely.
The rules that decide it
- Any workplace with one employee is covered. Health insurance applies to every workplace employing at least one worker. Unlike overtime premiums, which depend on whether a workplace has five or more employees, there is no small-business exemption here.
- Foreign employees are compulsory members. NHIS guidance says foreigners registered with immigration who work at a covered workplace are subscribed compulsorily.
- The start date is the hiring date. For a foreign worker, workplace eligibility begins on the day the person becomes an employee of the covered workplace.
- The report is due within 14 days. The employer files an acquisition report (직장가입자 자격취득 신고서) with NHIS within 14 days of that date.
- Employee consent is not a factor. Enrollment is the employer's duty regardless of whether the worker agrees.
Who is not a workplace subscriber
The main exclusions relevant to teaching are part-time workers whose contracted hours are under 60 a month, and daily workers employed for less than a month. A full-time E-2 teacher on a standard academy contract is far above that line. A teacher doing a few hours a week of private-school or after-school work on the side is a different case, and the hours in each contract are what decide it.
Practical tip: A probation clause that says insurance "starts after three months" does not change the legal start date. The guide to probation clauses explains why enrollment follows the employment relationship rather than the probation label.
Opting out with private insurance
There is a narrow route out of workplace enrollment for foreigners. A foreign employee can apply for exclusion by proving equivalent medical coverage, either under a foreign law or foreign insurance policy, or under a contract with the employer. If the application is filed within 14 days of the acquisition report, the teacher is treated as never having been enrolled. Filed later, exclusion runs from the application date.
For most E-2 teachers this route is rarely worth pursuing. The cover has to be shown to be equivalent, the paperwork is the employer's to file, and it removes the teacher from a system that most clinics and pharmacies are built around.
What it costs in 2026
Short answer: For 2026 the health insurance rate is 7.19 percent of monthly wages, split equally, so a teacher pays 3.595 percent and the academy pays 3.595 percent. Long-term care insurance (장기요양보험) is charged on top at 0.9448 percent of wages for 2026, also split equally. Together, the teacher's share comes to a little over 4 percent of gross wages.
The 2026 health rate rose from 7.09 percent, the level it had held since 2023. The long-term care rate rose from 0.9182 percent in 2025, which works out to 13.14 percent of the health insurance premium.
| 2026 rate | Total | Teacher pays | Academy pays |
|---|---|---|---|
| Health insurance | 7.19 percent of wages | 3.595 percent | 3.595 percent |
| Long-term care insurance | 0.9448 percent of wages | 0.4724 percent | 0.4724 percent |
| Combined | 8.1348 percent of wages | 4.0674 percent | 4.0674 percent |
A worked example
For a teacher whose reportable monthly wage is ₩2,500,000, the 2026 figures come out approximately as follows. NHIS applies its own rounding, so payslips may differ by a few won.
| Item | Calculation | Teacher's share |
|---|---|---|
| Health insurance | ₩2,500,000 × 7.19 percent = ₩179,750, halved | About ₩89,875 |
| Long-term care insurance | ₩2,500,000 × 0.9448 percent = ₩23,620, halved | About ₩11,810 |
| Total monthly deduction | About ₩101,685 |
The academy pays a matching amount that does not appear as a deduction. These are illustrative figures. The base is the wage as defined for insurance purposes, which excludes severance pay and non-taxable earned income, so a salary that includes a non-taxable allowance will produce a slightly lower premium.
Cap, floor, and timing
- Monthly cap and floor. The wage-based premium has a monthly ceiling of ₩9,183,480 and a floor of ₩20,160, figures far from any ordinary teaching salary.
- The first deduction may come later than expected. Premiums are collected from the month after eligibility begins, unless eligibility begins on the 1st of a month, in which case that month is charged. A teacher who starts on March 10 will normally see the first premium for April.
- April adjustment. Employers report each worker's total pay for the previous year by March 10, NHIS recalculates the year's premiums, and the difference appears on the April bill. A raise during the year usually produces an extra charge in April. A pay cut produces a refund. Where the extra amount is at least one month's premium, the employer can apply to spread it over up to 12 installments.
Practical tip: The deduction labeled 건강보험 on a payslip should be close to 3.595 percent of the wage, and the one labeled 장기요양 should be close to 13.14 percent of the health line. A deduction far above those numbers usually means the employer share is being taken from the teacher as well, which the law does not allow.
Regional enrollment and the six-month rule
Short answer: Since July 16, 2019, foreigners who have lived in Korea for six months become regional subscribers automatically unless they are already covered through an employer or as a dependent. E-2 is one of the listed statuses. Regional premiums are paid entirely by the member, and for most foreigners they cannot fall below the national average premium.
For a teacher, this rule usually comes into play in a gap: between academies, after a contract ends while the visa is still valid, or when an employer never enrolled them.
How the six months are counted
- The count starts from the first entry into Korea.
- Short trips abroad totaling no more than 30 days within the six months still count as time in Korea.
- Regional eligibility begins the day after the six-month point.
- A teacher who leaves an academy job after more than six months in Korea, and stays on, moves from workplace to regional status rather than becoming uninsured.
How the premium is set
Regional premiums are calculated from a point score based on household income, property, and cars, multiplied by a value per point that is ₩211.5 for 2026. For foreign regional members other than permanent residents (F-5) and marriage migrants (F-6), NHIS charges the average premium if the calculated amount is lower. In practice, a newly arrived teacher with no Korean property will usually pay that floor.
The exact won figure for the 2026 foreign minimum is not published in a form this guide could confirm, and secondary sources give conflicting numbers. The amount appears on the NHIS bill, and 033-811-2000 can confirm it.
Payment in advance, and what happens when it is missed
Foreign regional members pay ahead. Each month's premium is due by the 25th of the previous month. If a premium is still unpaid by the 25th of the following month, benefits stop from the month after that.
There is a second consequence. Since August 2019, the Ministry of Justice has received arrears data from NHIS and restricts the permitted stay of foreigners with unpaid premiums when they apply to extend. Unpaid premiums can therefore mean a shorter permitted stay than the teacher applied for.
Practical tip: Anyone moving to regional status should set up automatic payment straight away. A bill sent to an old address is the usual way an arrears record starts, and that record can surface at the next visa extension.
What the insurance covers and what a visit costs
Short answer: National Health Insurance covers diagnosis, tests, treatment, surgery, hospitalization, prescriptions, rehabilitation, and preventive care, on the same terms for foreigners as for Koreans. The patient pays a percentage, called the copayment (본인부담금), which rises with the size of the hospital. At a neighborhood clinic it is 30 percent of the covered cost.
The copay structure is deliberate. It pushes routine care toward small clinics and keeps large university hospitals for serious cases.
| Where care is received | Patient copayment |
|---|---|
| Clinic (의원), outpatient | 30 percent |
| Hospital (병원), outpatient | 40 percent |
| General hospital (종합병원), outpatient | 50 percent in a city dong, 45 percent in an eup or myeon |
| Tertiary hospital (상급종합병원), outpatient | 60 percent |
| Pharmacy, prescription medicine | 30 percent |
| Inpatient stay | 20 percent, with lower rates for registered cancer and rare disease patients |
The annual ceiling
Korea caps the copayments a member pays in a year. Anything above the ceiling is reimbursed by NHIS. The ceiling depends on the premium bracket. For 2026 it runs from ₩900,000 a year for the lowest decile to ₩8,430,000 for the highest, for hospital stays of up to 120 days.
The ceiling does not cover everything. Non-covered services (비급여), selectively covered services, upgraded hospital rooms, dental implants, and some other items are excluded, so a large bill made up mostly of non-covered items is not protected by it.
What is not covered
Non-covered services are paid in full by the patient, and they also sit outside the annual ceiling. The NHIS list of items excluded from the ceiling includes dental implants and upgraded two- and three-bed hospital rooms, alongside non-covered and selectively covered services generally. Asking "보험 돼요?" (is this covered?) before agreeing to a procedure or test is the simplest way to avoid surprises.
Free checkups and other benefits
- General health checkup. Free for members. Office workers are eligible every two years and non-office workers every year.
- Cancer screening. Six types of cancer screening are offered, generally with a 10 percent copay, and no copay for colorectal and cervical cancer screening.
- Pregnancy and childbirth. A voucher of ₩1,000,000 per pregnancy, or ₩1,400,000 for multiple births, is available to members and dependents.
Practical tip: For a cold, an ear infection, or a minor injury, a clinic (의원) is both cheaper and faster than a hospital. Going straight to a university hospital without a referral means a 60 percent copay and usually a long wait.
The card, the app, and the English line
Short answer: NHIS issues a health insurance certificate (건강보험증) once a member is enrolled, but since May 20, 2024 hospitals and clinics must check the patient's identity at every visit, so the alien registration card has become the document that matters. A mobile health insurance card is also accepted. English help is available on 1577-1000, extension 6, or directly on 033-811-2000.
Many teachers wait for a paper card that never seems to arrive. In practice, an enrolled teacher with an alien registration card can be treated at the insured rate without one.
ID at the clinic
The identity check was introduced to stop people using someone else's insurance. Accepted documents include the alien registration card, a driver's license, and the mobile health insurance card or QR code. Reports at the time noted that pharmacies were not included in the requirement. Children, pregnant women, and some other groups are exempt.
The apps
NHIS runs The건강보험 app and a separate mobile health insurance card app (모바일 건강보험증), which shows a QR code at reception. Both depend on Korean mobile identity verification. How smoothly that works for foreign residents depends on how the phone plan is registered, so the physical alien registration card is the reliable fallback.
Phone and in person
| Line | Number | Notes |
|---|---|---|
| NHIS main call center | 1577-1000 | Press extension 6 for foreign language consultation |
| NHIS foreign language line | 033-811-2000 | English, Chinese, Uzbek, Vietnamese; 9am to 6pm |
| From overseas | +82-33-811-2001 | Useful after leaving Korea |
| Health and welfare call center | 129 | Broader health system and welfare questions |
Practical tip: The document that settles any question about enrollment is the certificate of eligibility history (자격득실확인서). It lists every period of workplace, regional, and dependent coverage with dates and employer names. The foreign language line can explain how to obtain it, and it is also useful as proof of employment when a bank asks.
If the academy has not enrolled a teacher
Short answer: Failing to report a worker does not take away the worker's entitlement. NHIS can enroll the worker on its own authority with effect from the real start date, collect the unpaid premiums from the employer, and impose penalties under the National Health Insurance Act. The teacher's practical job is to find out early and get the record corrected.
The risk for the teacher is mostly about timing. A teacher who has been in Korea for six months without workplace enrollment becomes a regional subscriber, paying the full premium alone, and may not notice until a bill arrives.
Signs that enrollment has not happened
- No 건강보험 or 장기요양 deduction on payslips after the first full month.
- A clinic says the teacher is not on the system, or charges the uninsured price.
- A regional premium bill arrives at home while the teacher is employed full time.
- The academy says insurance "starts after probation" or "after the visa is finished."
Step by step
- Check the payslip. A health insurance and long-term care line should appear by the month after the start date at the latest.
- Get the certificate of eligibility history. It shows whether, and from what date, the academy reported the teacher.
- Ask the academy in writing. A short message asking for the date the acquisition report was filed is often enough. Many cases are administrative delays rather than refusals.
- Call 033-811-2000. NHIS can confirm the record and explain how to report an unreported workplace.
- Report the workplace if needed. NHIS can enroll the worker retroactively to the real start date.
- Keep the evidence. Contract, payslips, bank deposit records, and messages about the start date establish when employment actually began.
- Call 1350 for the wider picture. An academy that skipped health insurance has often skipped pension and employment insurance as well, and the labor ministry line can advise on all of it.
Practical tip: If the academy deducted premiums from salary but never reported the teacher, that is a more serious problem than a late filing, because money was withheld for something that was not paid. Payslips showing the deduction are the key evidence.
Adding a spouse or children as dependents
Short answer: A workplace subscriber can add a spouse, children, parents, and in limited cases unmarried siblings as dependents (피부양자), who are covered without paying a separate premium. Since April 3, 2024, foreign dependents generally need six months of residence in Korea first, but spouses and children under 19 are exempt and can be added on arrival.
For a teacher who arrives with family, this is the difference between one premium covering everyone and each adult paying the regional rate.
| Requirement | What NHIS checks |
|---|---|
| Relationship | Spouse, lineal ascendants or descendants (including the spouse's), and certain unmarried siblings |
| Income | Total income under ₩20,000,000 a year, with stricter limits for business and rental income |
| Property | Property tax base under ₩540,000,000, or under ₩900,000,000 with annual income of ₩10,000,000 or less |
| Residence | Six months in Korea, except spouses and children under 19 |
| Documents | Alien registration card, plus a foreign family or marriage document with an apostille or foreign ministry confirmation and a notarized Korean translation |
Foreign documents must have been confirmed or apostilled within nine months, and the employer files the dependent report. Because NHIS cannot check foreign family registers on its own, supporting documents are generally needed again whenever eligibility changes.
Practical tip: An apostilled marriage certificate and children's birth certificates take weeks to obtain in most home countries. Getting them before departure avoids a period in which a spouse is uninsured or billed as a regional member.
Changing jobs, gaps, and leaving Korea
Short answer: Workplace coverage ends the day after employment ends, and the employer must report the loss within 14 days. A teacher who stays in Korea after more than six months of residence moves to regional status. On final departure, coverage ends with the permitted stay or the day after leaving, and only overpaid premiums are refunded. Unlike the National Pension, there is no lump-sum refund.
Between academies
When one job ends and the next starts within days, the new academy simply reports a new acquisition. When there is a gap, the teacher may be billed as a regional member for the months in between. That bill is easy to overlook while moving apartments and transferring visas, and it is exactly the kind of small arrears that causes trouble at the next extension.
Final settlement with the employer
When employment ends, the employer recalculates the premiums for the final period and settles the difference with the teacher before settling with NHIS. That adjustment, positive or negative, normally appears on the last payslip.
Leaving Korea
- Regional members lose eligibility the day after the permitted stay ends, or the day after departure where the stay is still valid and they remain abroad for a month or more.
- Prepaid premiums for months after eligibility ends are overpayments and can be refunded. From January 2026, the government's National Secretary (국민비서) alert service notifies people of health and pension premium refunds.
- Unpaid premiums do not disappear on departure. The arrears data shared with immigration can restrict the stay granted on a later return.
Practical tip: Before a final flight, a five-minute call to 033-811-2000 to confirm that nothing is outstanding and to ask about any refund is worth more than any other single step in this guide. For the pension refund, which works very differently, see the National Pension guide.
For academies
Hagwon Hub is read by academy operators as well as teachers, and nearly every health insurance dispute traces back to a missed or delayed report.
- File within 14 days of the start date. Eligibility runs from the hiring date regardless of when the report is filed, so a late report only builds up retroactive premiums and penalty exposure.
- Do not tie enrollment to probation or visa processing. Neither changes the start date, and a contract clause saying otherwise will not hold.
- Do not accept a teacher's request to skip enrollment. Enrollment is compulsory regardless of consent. The only lawful exit is the foreign-coverage exclusion, which needs proof of equivalent cover and must be filed within 14 days to apply from the start.
- Deduct only the employee half. Withholding the employer's 50 percent from salary is not permitted, and the employer must tell the employee the amount deducted.
- Explain the April adjustment. A one-line note in March that April payslips will include an annual health insurance settlement prevents most confused messages.
- Report departures within 14 days. A late loss report leaves the teacher on the books and makes the final settlement messier for both sides.
- Use the 4 major insurances portal. The combined social insurance site (4insure.or.kr) handles health, pension, employment, and industrial accident reports in one filing.
Common problems and what causes them
| Problem | Cause | What works |
|---|---|---|
| No health insurance deduction after two months | Academy has not filed the acquisition report | Check the eligibility history certificate and ask for the report date in writing |
| Regional bill arrives while employed full time | Six months passed without workplace enrollment | Have the academy report retroactively; NHIS adjusts the regional record |
| Clinic charges the uninsured price | Enrollment not yet processed, or no ID shown | Show the alien registration card; keep receipts and ask NHIS what can be adjusted once enrollment is backdated |
| Large extra deduction in April | Annual premium settlement after a pay increase | Normal; installments can be requested through the employer |
| Deduction far above 4 percent of wages | Employer share being passed to the teacher | Compare against the 2026 rates and raise it with the academy, then 1350 |
| Spouse refused as a dependent | Missing apostille or Korean translation | Obtain apostilled documents with notarized translation |
| Visa extension shortened | Unpaid regional premiums from a job gap | Pay the arrears and set up automatic payment |
| Expensive bill despite insurance | Non-covered services or a tertiary hospital visit | Ask whether a treatment is covered; start at a clinic |
Frequently asked questions
Do E-2 teachers have to join Korean national health insurance?
Yes. A teacher employed by an academy is a compulsory workplace member from the start date, and a foreign resident without an employer is enrolled as a regional member after six months.
How much is health insurance for foreigners in Korea in 2026?
For employees, 7.19 percent of wages split equally, plus long-term care insurance at 0.9448 percent of wages split equally. The teacher's share is a little over 4 percent of gross wages in total.
Does the academy have to pay half of the health insurance?
Yes. The law sets a 50/50 split between employee and employer for workplace members, and the academy may only deduct the employee half from salary.
Is there health insurance during probation?
Yes. Workplace eligibility begins on the date of employment, and a probation period does not delay it.
Can a teacher use private insurance instead?
Only by applying for exclusion with proof of equivalent coverage under foreign insurance or an employer contract. Filed within 14 days of enrollment, the exclusion applies from the start.
What does a clinic visit cost with Korean health insurance?
The patient pays 30 percent of the covered cost at a clinic and 30 percent at the pharmacy, rising to 60 percent for outpatient care at a tertiary hospital. Non-covered items are paid in full.
Is there an English line for health insurance in Korea?
Yes. Call 1577-1000 and press extension 6, or dial 033-811-2000 directly, from 9am to 6pm. From overseas the number is +82-33-811-2001.
Can health insurance premiums be refunded when leaving Korea?
Only overpaid premiums, such as months paid in advance after coverage ends. There is no lump-sum refund like the National Pension's.
Can a spouse be covered under the teacher's insurance?
Yes, as a dependent, if income and property limits are met and family documents are apostilled and translated. Spouses and children under 19 are exempt from the six-month residence requirement introduced on April 3, 2024.
What happens if health insurance premiums go unpaid?
For regional members, benefits stop if a premium remains unpaid past the following month's deadline, and the Ministry of Justice can restrict the permitted stay at the next extension.
Why did health insurance go up on the April payslip?
NHIS recalculates the previous year's premiums using actual pay, and the difference is charged or refunded on the April bill.
The short version
Every E-2 teacher at an academy is a workplace member of National Health Insurance from the first day of work. The academy must report the teacher within 14 days, and probation does not change that.
For 2026 the rate is 7.19 percent of wages plus 0.9448 percent for long-term care, each split equally, so the teacher pays a little over 4 percent. Coverage matches what Koreans receive, with a 30 percent copay at clinics and pharmacies and an annual ceiling on covered costs.
The real risks are gaps. A teacher who goes unreported, changes jobs, or stays on after a contract may be billed as a regional member at the full average premium, and unpaid regional premiums can follow a teacher into the next visa extension.
The single most useful action is to check the first full payslip for a health insurance line and, if it is missing, ask the academy that week for the date it filed the report.
